Abstract:

In the European population, basal celi carcinoma (BCC) represents the most prevalent malignant tumour of the eyelid skin, accounting for over 90% of cases. The remainder of cases are attributed to sąuamous celi and sebaceous gland carcinomas. The standard treatment entails the surgical excision of the lesion followed by reconstruction. The objective of this study is to ascertain the most efficacious surgical management strategies for eyelid skin cancer through a retrospective analysis of data from the Department of Cranio-Maxillofacial Surgery at the Jagiellonian University Medical College (2002-2020). Factors including tumour advancement, histopathological diagnosis and location were taken into account in order to identify the optimal reconstruction method. The principal findings indicate that non-radical excision of BCC markedly elevates the risk of local recurrence. A narrow resection margin was not found to be associated with an increased risk of recurrence. However, tumours with advanced stages, affecting multiple anatomical areas, and older patients’ age were identified as factors that may contribute to an elevated risk of orbital invasion. Tumours with a diameter of 21-30 mm exhibited a more than 15-fold higher risk exenteration. The study revealed that tumours with greater advancement resulted in a higher incidence of regional flap reconstruction. No significant correla ; tion was identified between reconstruction methods and complications. Achieving optimal aesthetic and functional outcomes freąuently necessitates the performance of multiple surgical procedures.
