Title: Antibiotic stewardship and barriers to effective diagnosis and treatment of hospital- and community-acquired infections : |b monitoring multidrug resistance microorganisms prevalence and antibiotic consumption patterns

Abstract:

Introduction The aim of the study was effective empirical antibiotic therapy, including fluoroquinolones (FQ), in the context of data from ECDC surveillance systems and local data, and identification of barriers that may interfere with the diagnostic and therapeutic process of Gram-negative rod infections (GNB). Materials and methods The analysis of FQ consumption and GNB sensitivity to FQ in the EU/EEA countries and in Poland in 2012-2021 using the „resistance rate” parameter (RR%). A similar assessment was carried out in the study with the intervention consisting in the introduction of restrictions on the use of FQ in 2023 vs. 2018-2019 and 2022: the consumption of FQ, total antibiotics, the share of FQ in total consumption and RR% GNB on FQ were analyzed. In addition, the prevalence of genes encoding selected carbapenemases was investigated with commercially available screening tests in Acinetobacter baumannii (CRAB) isolated from infection cases. Results and conclusions The consumption of FQ in EU/EEA countries decreased and remained stable in Poland. In Poland, the prevalence of FQ-resistant GNBs increased 2-fold compared to EU/EEA countries, and RR% showed an upward trend. The intervention study found a 10-fold reduction in the share of FQ consumption, an increase in overall antibiotic consumption, and a reduction in RR% of GNB analysis per FQ. FQ resistance among the GNB ; s analyzed exceeded the value allowing the use of FQ in empirical therapy. Available screening tests to identify carbapenemase producers do not detect the carbapenem resistance genes found in the CRAB strains.
